Project objectives

COORDINARE is responsible for strengthening and building capacity in general practice and primary care services within its region. It commissioned this work to help its general practices implement the PCMH model of care and to establish stronger supports in this transformation.

HPA’s task was to identify barriers and enablers of implementing the PCMH model of care. The review covered papers published from January 2013 to January 2017.

Our approach

We identified 1,463 titles through the search strategy and included 170 papers in the synthesis.

A range of barriers and enablers were identified, documented and organised by components required to transition to a PCMH.

Outcome

COORDINARE used the findings from the evidence check to build capacity within its region and help its general practices to implement the PCMH model.
